Ontario Health Network Unifies Patient Data Across 14 Clinics

A regional health network in Ontario operated 14 clinics, each running different EHR systems with no unified view of patient data. Clinicians lacked access to complete patient histories when patients moved between facilities, leading to duplicate testing, medication errors, and poor care coordination. The network needed to comply with PHIPA regulations while creating a unified patient data platform.

Zaha implemented a Health Information Exchange (HIE) platform built on HL7 FHIR standards, creating a centralized patient data repository that integrates with each clinic's existing EHR system. We developed custom FHIR adapters for three different EHR platforms (Epic, Cerner, and MEDITECH) to normalize and synchronize patient records in real time. The solution included a clinical data analytics dashboard that provides population health insights, a secure patient portal for accessing health records, and a consent management system that ensures PHIPA compliance. Role-based access controls and comprehensive audit logging were implemented to meet Ontario Health Teams requirements.
